1. You may continue treatment with any orthodontic provider of your choice.
2. If you prefer to continue treatment elsewhere, please complete the Authorization for Release of Records and return it to our office before October 15. Upon receipt of your completed authorization, we will promptly transfer your records to the provider you designate.
